Nutrition for Longevity (N4L) proudly serves the Clinton, MD community by providing Home Delivered Meals and Medically Tailored Meals to individuals eligible under Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS), and the Older Americans Act. Our dedicated service focuses on delivering nutritious meals crafted by a Registered Dietitian, designed to meet the specific dietary needs for over 36 medical conditions such as Chronic Kidney Disease and Congestive Heart Failure. Our meal options cater to dietary restrictions, ensuring they are free from dairy, peanuts, gluten, and pork. For Medicaid and Older Americans Act participants, meal delivery eligibility can vary, and we recommend consulting with your Case Manager or local Area Agency on Aging for assistance. Medicare Advantage beneficiaries can check their plan for meal benefits related to post-discharge care. For those without eligible coverage, our self-pay program offers affordable home-delivered meals at just $7.75 each, complete with the freedom to choose your meals. In addition, N4L provides Medical Nutrition Therapy and Nutrition Counseling services, helping to promote overall health and well-being.
