Located in Clarksburg, MD, Diabetes and Heart Health Services is dedicated to empowering individuals to prevent and effectively manage diabetes and heart disease. Recognizing the close connection between these two conditions, especially among African Americans, our comprehensive program offers essential resources including Chronic Disease Management Classes, Self-management Counseling, and health screenings.
