Located in Chevy Chase, MD, Diabetes and Heart Health Services is dedicated to empowering individuals to prevent and manage diabetes and heart disease through comprehensive education and support. Recognizing the critical connection between these two conditions, our program offers essential resources including Chronic Disease Management Classes, self-management counseling, and health screenings tailored to the needs of our community. We are committed to raising awareness about diabetes and heart disease through engaging events such as health fairs and presentations throughout Montgomery County. Funded by Montgomery County DHHS and implemented by McFarland & Associates, Inc., our mission is to provide impactful, practical assistance to those affected by these chronic diseases, fostering a healthier community for everyone.
