Diabetes and Heart Health Services

Located in Bethesda, MD, Diabetes and Heart Health Services is dedicated to empowering the community through education focused on the prevention and management of diabetes and heart disease. With a significant correlation between these two conditions—especially affecting the African American community where about 33% of adults with diabetes also contend with heart disease—our program offers essential resources including Chronic Disease Management Classes, support meetings, self-management counseling, and health screenings. We actively engage in outreach throughout Montgomery County by participating in walks, health fairs, and informative presentations, ensuring that crucial information about diabetes and heart health reaches those who need it most. Funded by the Montgomery County DHHS and implemented by McFarland & Associates, Inc., our mission is to provide practical tools and support for healthier living.
