Diabetes Self Management Education Program (DSME)

Located in Stoughton, MA, the Diabetes Self Management Education Program (DSME) is committed to empowering individuals living with diabetes through comprehensive, high-quality education tailored to your specific needs. Our program, led by experienced registered nurses and dietitians, starts with a thorough private assessment to help guide your learning journey. We offer a series of four engaging, two-hour group classes each month, covering essential topics such as diabetes fundamentals, healthy meal planning, physical activity integration, medication management, blood glucose monitoring, and strategies for preventing both acute and chronic complications. Additionally, we address critical aspects of stress management and problem-solving. Our services are accessible through MassHealth, Medicare, and commercial insurance, ensuring that you receive the support you need to manage your diabetes effectively. To join our program, please ask your physician to submit a completed diabetes education order of medical necessity to our office.
