The Adult Services Outreach Program in Springfield, MA, is dedicated to empowering adults with developmental disabilities in the greater Westfield and West Springfield area. Our mission is to help individuals cultivate the self-reliance and independent living skills essential for thriving in their communities. Through personalized case management and tailored support, we assist participants in navigating daily living skills, including health and hygiene maintenance, money management, nutrition, and social development. Our outreach team fosters essential connections, promoting housing acquisition and retention while facilitating access to medical, dental, and mental health services. All services are provided at no cost to participants, with a few optional fees for community activities. Referrals to our program are made exclusively through the Westfield/Springfield Area Office of DDS, ensuring a streamlined process for those seeking support in building their support networks and enhancing their quality of life.
