Diabetes Self Management Education Program (DSME)

At the Diabetes Self Management Education Program (DSME) in Mansfield, MA, we empower individuals with diabetes through high-quality, tailored education delivered by experienced registered nurses and dietitians. Our program begins with a thorough private assessment to identify your unique learning needs, followed by a series of four comprehensive two-hour group classes offered monthly, both during days and evenings. Participants will gain essential knowledge about diabetes basics, healthful meal planning, physical activity integration, medication management, and how to monitor and interpret blood glucose levels. We also focus on preventing complications and developing effective stress management and problem-solving strategies. Our services are accessible through MassHealth, Medicare, and various insurance providers. To join, please have your physician fax a completed diabetes education order of medical necessity to our office for registration.
