Diabetes HELP

Diabetes HELP is dedicated to enhancing the health and quality of life for Louisiana Medicaid recipients living with diabetes. Our program offers comprehensive diabetes self-management education tailored to empower individuals through quality education and community support. Participants engage in small group sessions where they learn essential skills for managing their condition, covering vital topics such as nutrition, physical activity, medication management, and coping strategies. We foster collaboration among patients, pharmacists, physicians, and healthcare providers to create a robust support network. While we complement regular medical care, our focus is on equipping participants with the practical tools they need to navigate their diabetes confidently. Located in Zachary, Louisiana, we invite community members to join us in taking proactive steps towards better diabetes management.
