Diabetes HELP

Located in Youngsville, LA, the Diabetes HELP program is dedicated to enhancing the health and quality of life for Louisiana Medicaid recipients living with diabetes. We provide essential diabetes self-management education and support designed to foster collaboration among patients, pharmacists, physicians, and healthcare professionals. Through small group sessions, participants gain vital skills and knowledge to manage their condition effectively, covering crucial topics such as diet, exercise, medication review, and understanding blood sugar levels. Our program complements traditional physician care by empowering individuals with the understanding and tools necessary to manage their diabetes, all rooted in a framework established by the national Diabetes Initiative.
