Diabetes HELP

Located in Ville Platte, LA, the Diabetes HELP program is dedicated to enhancing the health and quality of life of Louisiana Medicaid recipients living with diabetes. Through our comprehensive diabetes self-management education program, we provide essential education and support that empowers participants to better manage their condition. Our approach fosters collaboration among patients, pharmacists, physicians, and other healthcare providers, ensuring holistic care. In small group settings, individuals learn vital diabetes self-management skills, discussing their experiences with licensed pharmacists and peers. Our educational sessions cover crucial topics including diabetes fundamentals, nutrition, physical activity, blood sugar monitoring, and medication management. While we do not replace regular physician care, we focus on equipping participants with the tools necessary for effective self-management, building a stronger foundation for their health and wellbeing in the community.
