Located in Tickfaw, LA, Cancer Transitions is dedicated to supporting cancer survivors who have recently completed treatment by providing a comprehensive, evidence-based program tailored to their needs. Our mission is to empower individuals, regardless of when they completed their treatment, with essential resources focused on nutrition, exercise, emotional well-being, and medical management. Designed for those who are within 24 months post-treatment but beneficial for all survivors, our program fosters a nurturing environment where survivors can find guidance and support as they transition into the next chapter of their lives.
