Diabetes HELP is dedicated to enhancing the health and well-being of Louisiana Medicaid recipients living with diabetes in Pineville, LA, by offering comprehensive diabetes self-management education and supportive resources. Our program fosters better health outcomes through collaborative care among patients, licensed pharmacists, and healthcare professionals, ensuring participants receive strong guidance and encouragement. In our engaging small group sessions, individuals gain valuable self-management skills, tackling essential topics such as diabetes overview, nutrition and diet, physical activity, and medication management. While not a substitute for regular physician visits, our educational approach empowers individuals to take control of their diabetes, improving their quality of life and managing symptoms effectively. We are proud to implement our framework based on the Diabetes Initiative by the Robert Wood Johnson Foundation, focusing on the essential need for informed self-management in diabetes care.
