Diabetes HELP

Located in Patterson, LA, the Diabetes HELP program is dedicated to enhancing the health and well-being of Louisiana Medicaid recipients living with diabetes. Our mission centers around empowering individuals through comprehensive diabetes self-management education, fostering collaboration among patients, healthcare providers, and pharmacists to create a supportive environment for disease management. Participants engage in small group settings where they learn essential skills such as nutrition and diet, coping strategies, physical activity, and recognizing the signs of altered blood sugar. By offering educational sessions and interactive discussions, we not only equip participants with the tools needed to manage their diabetes effectively but also advocate for ongoing physician care. Our program, developed within the framework of the national Diabetes Initiative by the Robert Wood Johnson Foundation, emphasizes the importance of self-management as a pathway to improved health outcomes and quality of life for those affected by diabetes.
