Diabetes HELP

Diabetes HELP is dedicated to enhancing the health and well-being of Louisiana Medicaid recipients living with diabetes in New Iberia, LA. Our mission is to empower participants through our diabetes self-management education program, which fosters collaboration among patients, healthcare providers, and pharmacists. By providing small group sessions, we create a supportive environment where individuals can learn essential diabetes management skills, engage in discussions with licensed pharmacists, and share experiences with peers. Our educational sessions cover vital topics including nutrition, physical activity, medication management, and understanding the signs of blood sugar variations. While we complement regular medical care, our focus is on equipping participants with the knowledge and tools to manage their diabetes effectively, ensuring they lead healthier, more fulfilling lives. The program is built on a strong foundation established by the Diabetes Initiative from the Robert Wood Johnson Foundation, reinforcing the importance of self-management in diabetes care.
