Diabetes HELP

Diabetes HELP is dedicated to enhancing the health and quality of life for Louisiana Medicaid recipients living with diabetes. This program offers comprehensive diabetes self-management education, empowering participants through small group sessions where they can learn vital skills. Our curriculum covers essential topics such as nutrition, physical activity, understanding blood sugar levels, and medication management, while also encouraging collaboration among patients, pharmacists, physicians, and other healthcare professionals. By fostering an environment of support and education, Diabetes HELP works in tandem with regular physician care, helping individuals navigate their diabetes journey with confidence and knowledge.
