Diabetes HELP

Diabetes HELP, located in Houma, LA, is dedicated to enhancing the health and well-being of Louisiana Medicaid recipients living with diabetes through comprehensive education and support. Our diabetes self-management education program equips participants with essential skills in small group settings, fostering collaboration among patients, pharmacists, physicians, and healthcare teams. We focus on empowering individuals with knowledge about managing their condition, covering critical topics such as diabetes basics, coping strategies, nutrition, physical activity, recognizing symptoms of fluctuating blood sugar, potential long-term complications, and medication reviews. While we support participants in their journey, we emphasize that our program is meant to complement, not replace, regular physician care. Grounded in the framework established by the national Diabetes Initiative from the Robert Wood Johnson Foundation, Diabetes HELP remains committed to promoting effective self-management for a better quality of life.
