Diabetes HELP

Diabetes HELP is dedicated to enhancing the health and quality of life for Louisiana Medicaid recipients living with diabetes in Haughton, LA. Our program delivers essential diabetes self-management education and support, fostering collaboration among patients, pharmacists, physicians, and healthcare providers. In small group settings, participants gain vital skills for managing diabetes, engage in discussions with licensed pharmacists, and benefit from shared experiences with fellow members. Our comprehensive educational sessions cover critical topics such as diabetes fundamentals, coping strategies, nutrition, physical activity, blood sugar management, potential complications, and medication reviews. While we emphasize self-management education, we recognize the importance of regular physician care and aim to empower our participants with the knowledge to navigate their condition effectively. The Diabetes HELP program is built on the foundational framework of the Diabetes Initiative, a program backed by the Robert Wood Johnson Foundation, ensuring that we prioritize the support necessary for successful self-management.
