Diabetes HELP

Based in Gonzales, LA, the Diabetes HELP program is dedicated to enhancing the health and quality of life for Louisiana Medicaid recipients living with diabetes. Through our comprehensive diabetes self-management education program, we equip participants with essential skills and knowledge in a supportive small group environment. Our collaborative approach brings together patients, pharmacists, physicians, and healthcare professionals, fostering a community of care and understanding. Educational sessions cover vital topics such as diabetes fundamentals, nutrition, coping strategies, physical activity, and medication management, designed to empower individuals in their daily lives. While we emphasize the importance of regular physician care, our program provides valuable resources that promote effective self-management and long-term health, aligning with the principles established by the Diabetes Initiative, a national program of the Robert Wood Johnson Foundation.
