Diabetes HELP

Located in Franklin, LA, the Diabetes HELP program is dedicated to enhancing the health and quality of life for Louisiana Medicaid recipients living with diabetes. Through our comprehensive diabetes self-management education program, we empower individuals by providing essential knowledge and support in small group settings. Participants engage in meaningful discussions about their condition with licensed pharmacists and fellow program members, fostering a collaborative care approach that complements regular physician care. Our educational sessions are designed to equip individuals with vital diabetes management skills, addressing crucial topics such as nutrition, physical activity, coping strategies, and medication review.
