Diabetes HELP in Deridder, LA, is dedicated to enhancing the health and quality of life for Louisiana Medicaid recipients living with diabetes. Our program offers essential diabetes self-management education, empowering participants through small group sessions that emphasize practical skills and collaboration among patients, pharmacists, physicians, and other healthcare professionals. We provide a supportive environment where individuals can openly discuss their diabetes management challenges while receiving guidance on nutrition, physical activity, coping strategies, recognizing blood sugar fluctuations, and understanding long-term complications. While we encourage regular physician supervision, our educational sessions aim to equip participants with the tools they need to confidently manage their condition. Developed in alignment with the Diabetes Initiative from the Robert Wood Johnson Foundation, Diabetes HELP prioritizes effective self-management and comprehensive care.
