Diabetes HELP is dedicated to enhancing the health and quality of life for Louisiana Medicaid recipients living with diabetes in Belle Chasse, LA. Our program provides comprehensive diabetes self-management education, empowering participants with essential skills to manage their condition effectively in a supportive, small-group environment. We believe that collaboration is key to better health outcomes, which is why we facilitate meaningful interactions among patients, licensed pharmacists, physicians, and other healthcare professionals. Our educational sessions cover crucial topics such as diabetes basics, coping strategies, nutrition, physical activity, blood sugar awareness, potential complications, and medication management. Although we do not replace regular physician visits, our goal is to equip individuals with the knowledge and resources needed to navigate their diabetes journey with confidence and support.
