Home Delivered Meals, part of Nutrition for Longevity (N4L), is dedicated to providing nutritious, home-delivered meals tailored to the specific dietary needs of residents in South Bend, IN. Our program serves eligible clients enrolled in Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS) programs, and the Older Americans Act, ensuring that individuals receive meals designed by a Registered Dietitian to support their health and well-being. We cater to various medical conditions, including Chronic Kidney Disease and Congestive Heart Failure, with offerings that are free from dairy, peanuts, gluten, and pork. For those without qualifying health plans, we provide an affordable self-pay option at $7.75 per meal, allowing you to select every meal and delivery to suit your preferences. Additionally, we offer Medical Nutrition Therapy and Nutrition Counseling to promote overall health.
