Home Delivered Meals

Located in Portage, IN, Nutrition for Longevity (N4L) is dedicated to supporting individuals through our Home Delivered Meals and Medically Tailored Meals programs. We prioritize the health and well-being of eligible clients covered by Medicare Advantage, Medicaid Long-Term Services and Supports, or the Older Americans Act, as well as individuals seeking self-pay options. Our meals, crafted by Registered Dietitians, cater to various nutritional needs and medical conditions, including Chronic Kidney Disease and Congestive Heart Failure, and ensure dietary restrictions are met, being free from dairy, peanuts, gluten, and pork. For those eligible for government assistance, we encourage contacting your Case Manager or local Area Agency on Aging for meal delivery options. For Medicare Advantage beneficiaries, check your plan materials for meal benefits, particularly if on a PPO plan. For individuals not qualifying for assistance, our affordable self-pay program at $7.75 per meal offers the flexibility to choose meals and delivery times. In addition to meal services, N4L provides Medical Nutrition Therapy and Nutrition Counseling to further enhance our clients' health journey.
