Program of All-Inclusive Care for the Elderly (PACE)

Based in Osceola, Indiana, the Program of All-Inclusive Care for the Elderly (PACE) is dedicated to enhancing the lives of seniors with chronic care needs. Our mission is to deliver comprehensive medical and supportive services that empower older adults to maintain their independence and quality of life at home for as long as possible. We provide a wide range of services including adult daycare, nutrition counseling, therapies, medical and dental care, vision care, in-home support, and transportation. While enrollment requires certification for nursing home care, only a small percentage of our participants actually reside in a nursing facility. Should nursing home care become necessary, PACE seamlessly covers these services, ensuring consistent care coordination. We work closely with Medicare and Medicaid to provide eligible individuals with the inclusive support they need, affirming our commitment to comprehensive senior care in the community.
