Located in New Haven, IN, Nutrition for Longevity (N4L) is dedicated to providing vital Home Delivered Meals and Medically Tailored Meals to individuals eligible under Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS), or the Older Americans Act. Our meals, designed by a Registered Dietitian, cater to the diverse nutritional needs of clients with various health conditions, such as Chronic Kidney Disease, End Stage Renal Disease, and Congestive Heart Failure. We prioritize health and safety by ensuring that our meals are free from dairy, peanuts, gluten, and pork. For those eligible under Medicaid or the Older Americans Act, access to meal delivery varies depending on the program, and our team encourages beneficiaries to consult their Case Managers for specifics. Medicare Advantage beneficiaries may also find meal options within their post-discharge care plans. In addition, N4L offers a self-pay program for those wishing to enjoy the convenience of home-delivered meals at an affordable rate of $7.75 per meal, allowing you to choose your meals and delivery schedule. Beyond meal delivery, we provide Medical Nutrition Therapy and Nutrition Counseling to further support the well-being of our clients.
