Located in Monticello, IN, Nutrition for Longevity (N4L) is dedicated to enhancing the health and well-being of our community through our Home Delivered Meals program. We provide carefully crafted, Medically Tailored Meals specifically designed by Registered Dietitians to cater to various medical conditions including Chronic Kidney Disease, Congestive Heart Failure, and more. Our meals are available to individuals covered by Medicare Advantage plans, Medicaid Long-Term Services and Supports, and the Older Americans Act, as well as those seeking a self-pay option, ensuring everyone can access nutritious food that supports their health needs. Additionally, we offer Medical Nutrition Therapy and Nutrition Counseling services. If you’re unsure about your eligibility for home meal delivery, your Case Manager or local Area Agency on Aging can assist you in finding the right support. For self-pay individuals, our meals are conveniently priced at just $7.75 each, allowing you to choose your meals and delivery schedule.
