Program of All-Inclusive Care for the Elderly (PACE)

In Lowell, Indiana, the Program of All-Inclusive Care for the Elderly (PACE) is dedicated to supporting seniors with chronic care needs, ensuring they receive comprehensive medical and supportive services while maintaining their independence at home. Our holistic approach encompasses adult daycare, nutrition counseling, various therapies, meals, medical care, dental and vision care, in-home support, and transportation, bridging essential services to enhance quality of life. Although enrolling in PACE requires certification for nursing home care, our program allows 93% of participants to live outside institutional settings. Should the need for nursing home care arise, we seamlessly manage and fund that transition, ensuring continuity in care. PACE operates on a member reimbursement model through Medicare and Medicaid, making healthcare accessible for eligible seniors.
