Program of All-Inclusive Care for the Elderly (PACE)

The Program of All-Inclusive Care for the Elderly (PACE) in Hobart, IN, is dedicated to enhancing the lives of seniors with chronic care needs by providing a comprehensive range of medical and supportive services that empower them to maintain their independence at home.
