Program of All-Inclusive Care for the Elderly (PACE)

The Program of All-Inclusive Care for the Elderly (PACE) in Hammond, IN, is dedicated to enhancing the lives of seniors with chronic care needs by providing a comprehensive range of medical and supportive services designed to promote independence and well-being in the comfort of their homes. Our holistic approach includes adult daycare, nutrition counseling, various therapies, meals, medical, dental, and vision care, in-home support, as well as transportation services. While participants are certified to require nursing home care upon enrollment, it's noteworthy that only about seven percent actually reside in such facilities, highlighting our commitment to keeping seniors at home for as long as possible. Should the need for nursing home care arise, the PACE program continues to manage the participant's care, ensuring seamless support.
