Program of All-Inclusive Care for the Elderly (PACE)

In Granger, IN, the Program of All-Inclusive Care for the Elderly (PACE) is dedicated to empowering seniors with chronic care needs by providing a full spectrum of medical and supportive services, allowing them to maintain independence in their own homes for as long as possible. Our comprehensive services include adult daycare, nutrition counseling, therapies, meals, medical care, dental care, vision care, in-home care, and transportation, all tailored to meet the unique needs of each participant. While enrollment requires certification for nursing home care, only a small fraction of our participants reside in nursing facilities, as our program is designed to deliver care at home. Should nursing care become necessary, PACE continues to cover the costs and coordinate care seamlessly.
