To help us determine your eligibility for support programs and our sliding fee scale, we kindly ask you to bring necessary documentation to your initial intake appointment, such as identification, your social security number, insurance cards, and proof of income. We proudly accept Medicare and Medicaid, and provide translation services for medical appointments to accommodate Spanish speakers and the deaf community.
