Nutrition for Longevity (N4L) is dedicated to enhancing the health and well-being of our community in Demotte, IN through our Home Delivered Meals program. We provide nutritious and medically tailored meals designed by Registered Dietitians to individuals eligible for Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS), and the Older Americans Act. Our meals cater to 36 specific medical conditions, including Chronic Kidney Disease and Congestive Heart Failure, ensuring that clients receive the essential nutrients they need, free from dairy, peanuts, gluten, and pork. For those who don't qualify for government assistance, we offer an affordable self-pay option for just $7.75 per meal, making it easy to maintain a healthy diet from the comfort of home. N4L also provides Medical Nutrition Therapy and Nutrition Counseling services, further supporting our mission to deliver compassionate care through proper nutrition.
