Program of All-Inclusive Care for the Elderly (PACE)

At the Program of All-Inclusive Care for the Elderly (PACE) in Crown Point, IN, we are dedicated to enhancing the quality of life for seniors with chronic care needs by providing comprehensive medical and supportive services that foster independence in the comfort of their own homes. While PACE participants are certified for nursing home care, our aim is to keep approximately 93% of enrollees living at home.
