Located in Columbia City, IN, Nutrition for Longevity (N4L) is dedicated to enhancing the well-being of our community through our Home Delivered Meals program, designed specifically for clients eligible under Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS), or the Older Americans Act. Our medically tailored meals, crafted by a Registered Dietitian, cater to various health needs, ensuring that clients receive the essential nutrients necessary for their conditions, including Chronic Kidney Disease and Congestive Heart Failure. We prioritize dietary restrictions by offering meals that are free from dairy, peanuts, gluten, and pork. For those eligible for Medicaid or the Older Americans Act, delivery options are available, and assistance is easy to navigate through Case Managers or local aging services. We also support Medicare Advantage beneficiaries with options within specific plans. For individuals seeking our services without eligibility for government support, our affordable self-pay program offers customizable meal choices for just $7.75 per meal, ensuring everyone has access to nutritious food. In addition to meal services, N4L provides Medical Nutrition Therapy and Nutrition Counseling to further promote health and nutrition education in our community.
