Home Delivered Meals by Nutrition for Longevity (N4L) in Chesterton, IN, is dedicated to providing nutritious meals tailored to the needs of eligible clients, including those under Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS) programs, and the Older Americans Act. Our meal delivery service features meals specially curated by a Registered Dietitian to support overall health and well-being, addressing 36 specific medical conditions such as Chronic Kidney Disease and Congestive Heart Failure. We prioritize dietary restrictions by offering meals free from dairy, peanuts, gluten, and pork. For clients receiving Medicaid or Older Americans Act benefits, eligibility for meal delivery can vary, and we encourage collaboration with Case Managers to determine coverage. Our program is also available for eligible Medicare Advantage beneficiaries through specific plans. For those not covered by these options, we proudly offer a self-pay program at an affordable rate of $7.75 per meal, allowing complete flexibility in meal choice and delivery. Additionally, N4L provides Medical Nutrition Therapy and Nutrition Counseling to empower individuals in their health journey.
