Nutrition for Longevity (N4L) is dedicated to providing essential Home Delivered Meals and Medically Tailored Meals to individuals in Brazil, IN, ensuring that those eligible under Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS) programs, or the Older Americans Act have access to nutritious food designed to support their health and well-being. Our meals, crafted by a Registered Dietitian, cater to a variety of health needs including Chronic Kidney Disease, End Stage Renal Disease, and Congestive Heart Failure, and are free from dairy, peanuts, gluten, and pork. For those who qualify under Medicaid or the Older Americans Act, we facilitate meal delivery based on individual eligibility, and for Medicare Advantage beneficiaries, we work with several PPO plans to enhance their post-discharge support. Additionally, our self-pay option allows anyone seeking the convenience of home-delivered meals to select their meals for just $7.75 each, without government assistance. Beyond meal services, we also offer Medical Nutrition Therapy and Nutrition Counseling to further promote the health of our clients. Eligibility inquiries can be directed through case managers, Medicaid providers, or local Area Agencies on Aging.
