Home Delivered Meals

Located in Avon, IN, Nutrition for Longevity (N4L) is dedicated to enhancing health and well-being through our Home Delivered Meals and Medically Tailored Meals programs. We cater to eligible clients covered by Medicare Advantage, Medicaid Long-Term Services and Supports (LTSS), and the Older Americans Act, while also providing a self-pay option for those not covered by these programs. Our nutrient-rich meals are designed by Registered Dietitians to address the specific dietary needs associated with 36 medical conditions, including Chronic Kidney Disease, End Stage Renal Disease, and Congestive Heart Failure. We ensure our meals are free from dairy, peanuts, gluten, and pork to accommodate various dietary restrictions. For clients with Medicaid or Older Americans Act benefits, eligibility for meal delivery can vary; a Case Manager can provide guidance. Medicare Advantage beneficiaries should check their plan for meal benefits, as we currently serve PPO plan members. Our self-pay program offers an affordable option at just $7.75 per meal, allowing individuals to select their preferred meals for convenience and care. Additionally, we provide Medical Nutrition Therapy and Nutrition Counseling services to support our clients' health journeys.
