Located in Freeport, IL, Cancer Transitions is a compassionate program dedicated to helping cancer survivors who have completed their treatment. Designed for individuals within the first 24 months post-treatment, this evidence-based initiative also welcomes those further along in their survivorship journey. We offer vital resources including nutritional support, customized exercise plans, emotional health education, and medical management education, all aimed at empowering survivors to thrive in their ongoing journey of recovery and well-being.
