Maternal Child Health (MCH) Case Management Program

The Maternal Child Health (MCH) Case Management Program in Elk Grove Village, IL, is dedicated to supporting pregnant women and infants up to one year old through comprehensive case management services. Recognizing you as your baby's first teacher and advocate, our experienced case managers work alongside you to help achieve your goals for yourself and your family. We offer a wide range of services including pre- and post-natal care, healthcare scheduling with a Primary Medical Care Physician, assistance with KidCare and FamilyCare health insurance, and vital immunization information. Our program also provides education on well-child visits, customized care plans, home and face-to-face visits, and support with WIC appointments. In addition, we lend a hand with the Food Stamp Program, offer connections to community resources, and provide emergency assistance such as diapers, formula, and food from Firman’s Food Pantry. We host quarterly events including support groups and baby showers, distribute free clothing for families, and provide access to our Emergency Fund Program to help with urgent expenses like gas, utilities, and rent.
