Cancer Transitions is a supportive and evidence-based program located in Des Plaines, IL, designed for cancer survivors who have completed their treatment within the last 24 months, although it can also benefit those further along in their survivorship journey. Our mission is to empower individuals through comprehensive resources that focus on improving their overall well-being. We offer tailored nutritional support, customized exercise programs, emotional health education, and crucial guidance on medical management. Our dedicated team is here to help survivors navigate the challenges of post-treatment living, fostering a sense of community and resilience in the face of cancer.
