Health Risk Assessment Program

Located in Winter Garden, FL, the Health Risk Assessment Program, created by HHI, is dedicated to enhancing wellness among uninsured and under-insured adults at risk for chronic diseases. Through culturally and linguistically adapted health screenings, education, and personalized case management, we aim to improve health indicators such as Body Mass Index, blood glucose, blood pressure, pulse, and cholesterol levels. Our community health workers conduct thorough assessments and provide tailored health education based on individual screening results and health histories, empowering participants with knowledge about their risk factors for chronic diseases. To further support sustainable health improvements, participants receive one-on-one coaching and case management follow-ups, fostering the development of healthier lifestyle choices for long-term well-being.
