At Nutrition for Longevity (N4L) in Orange Park, FL, our mission is to provide essential food assistance through our Home Delivered Meals and Medically Tailored Meals programs. We cater to individuals eligible under Medicare Advantage, Medicaid Long-Term Services and Supports, and the Older Americans Act, as well as those seeking a self-pay option. Each meal is meticulously planned by a Registered Dietitian to meet the specific nutritional needs for general health and various medical conditions, including Chronic Kidney Disease and Congestive Heart Failure. Our meals are designed to be inclusive, free from dairy, peanuts, gluten, and pork, ensuring a safe dining experience for our clients. We encourage those with a Case Manager to check their benefits for meal delivery options. For Medicare Advantage beneficiaries, meal services may be available through select plans, while self-pay individuals can enjoy the convenience of customized home-delivered meals for just $7.75 each, plus a flat-rate shipping fee. In addition to our meal offerings, we provide Medical Nutrition Therapy and Nutrition Counseling, reinforcing our commitment to the overall well-being of our community members.
