Located in Longwood, FL, the Health Risk Assessment Program is dedicated to enhancing the wellness of uninsured and under-insured adults who are at heightening risk for chronic diseases. Our initiative provides accessible, culturally sensitive health screenings, education, and case management, aiming to significantly improve health indicators such as Body Mass Index, blood glucose levels, blood pressure, pulse, and cholesterol. Our skilled Community Health Workers conduct comprehensive assessments, allowing participants to better understand their health status and risk factors. Coupled with personalized health education informed by screening results and family health history, we empower individuals to take charge of their health.
