The Health Risk Assessment Program in Lake Mary, FL, is dedicated to enhancing wellness and improving health outcomes for uninsured and under-insured adults at risk for chronic disease. Through community-based, culturally sensitive health screening, education, and personalized case management, our mission is to empower individuals to take charge of their health. We conduct comprehensive screenings that assess critical health indicators such as Body Mass Index, blood glucose levels, blood pressure, pulse, and cholesterol for an accurate health risk profile. Following the assessment, our trained Community Health Workers provide tailored health education that addresses individual risk factors based on screening results and personal health history.
