Cancer Transitions is a supportive program located in Colchester, CT, designed specifically for survivors of any cancer diagnosis who have recently completed their treatment. While primarily focused on those within 24 months post-treatment, our resources are beneficial to all survivors at any stage of their journey. We offer essential services including nutritional support to help rebuild strength, customized exercise plans tailored to individual needs, emotional health education, and medical management guidance, all aimed at empowering participants to navigate their survivorship with confidence and resilience.
