Located in Fort Morgan, CO, Nutrition for Longevity (N4L) is dedicated to enhancing health and well-being through our Home Delivered Meals and Medically Tailored Meals programs, specifically designed for individuals covered by Medicare Advantage plans, Medicaid Long-Term Services and Supports, or the Older Americans Act. Our meals, thoughtfully crafted by a Registered Dietitian, cater to the nutritional needs of clients facing various medical conditions, including Chronic Kidney Disease and Congestive Heart Failure, and are free from common allergens such as dairy, peanuts, gluten, and pork. For those eligible, we provide meal delivery services through most Medicaid and Older Americans Act programs; clients can inquire through their Case Manager or local Area Agency on Aging. Additionally, Medicare Advantage beneficiaries may be able to access our meals as part of their post-discharge or chronic care support programs. For individuals seeking nutritious meals without insurance coverage, we offer an affordable self-pay option at $7.75 per meal, allowing you to select your meals and delivery schedule. In addition to meal services, N4L also provides Medical Nutrition Therapy and Nutrition Counseling, ensuring comprehensive support for every client on their journey to better health.
