Located in Cortez, CO, Nutrition for Longevity (N4L) is dedicated to enhancing the well-being of our community through our Home Delivered Meals and Medically Tailored Meals programs. We cater to eligible clients covered by Medicare Advantage plans, Medicaid Long-Term Services and Supports, and the Older Americans Act, as well as individuals seeking self-pay options for nutritious meal delivery. Our meals, expertly planned by a Registered Dietitian, are carefully crafted to meet specific nutrient requirements for a range of medical conditions, including Chronic Kidney Disease, End Stage Renal Disease, and Congestive Heart Failure. With a commitment to health, our meals are free from dairy, peanuts, gluten, and pork, ensuring they are suitable for diverse dietary needs. For those utilizing Medicaid or the Older Americans Act, eligibility for meal delivery may vary, so we encourage you to consult with your Case Manager or local Area Agency on Aging for assistance. Our program also supports Medicare Advantage beneficiaries with meals as part of their post-discharge or chronic care support, while our self-pay program offers affordability at just $7.75 per meal, giving you complete control over your meal selection and delivery. Additionally, N4L provides Medical Nutrition Therapy and Nutrition Counseling, helping clients make informed dietary choices tailored to their health needs.
