Type 2 Diabetes Nutrition Program

The Type 2 Diabetes Nutrition Program, based in San Diego, CA, is dedicated to improving the health and well-being of individuals managing Type 2 diabetes through the provision of free home deliveries of three nutritious meals each day, seven days a week. Our program focuses on medically tailored meals (MTM) and personalized medical nutrition therapy to enhance health outcomes and reduce healthcare costs within just three months. We work closely with healthcare providers to ensure that our meals meet basic nutritional needs for eligible participants. To join the program, a referral form must be signed by a healthcare provider. We prioritize the safety and effectiveness of our services and are unable to accept those living in facilities that provide more than seven meals weekly, individuals with certain cognitive or physical limitations, or anyone with severe food allergies.
