Our mission focuses on improving health outcomes for individuals with chronic kidney disease through individualized medical nutrition therapy interventions, which have been shown to enhance healthcare results and decrease costs significantly in just three months. To join this essential program, a referral form must be completed and signed by a healthcare provider. We aim to provide vital food assistance to those in need; however, participants must meet specific criteria, including not residing in a facility that provides more than seven meals weekly and having the capability to engage with our services effectively.
