Program of All-Inclusive Care for the Elderly (PACE)

Located in La Puente, CA, the Program of All-Inclusive Care for the Elderly (PACE) is dedicated to empowering seniors to age gracefully in the comfort of their own homes, while avoiding nursing home placements. By offering a comprehensive suite of essential services including nursing, personal care, physical and occupational therapy, and prescription pharmacy services, PACE ensures that participants receive the holistic care they require. Our program also includes recreational therapy, meals and nutritional counseling, audiology, social work, dental care, optometry, podiatry, and speech therapy. We prioritize individualized care with additional services such as care coordination, transportation, and assistance with Medicare enrollment, making it easier for eligible seniors to access the support they need. PACE is committed to working closely with seniors and their caregivers to evaluate eligibility and provide a seamless integration of services under Medi-Cal and Medicare.
